This form is provided for league secretaries to quickly and easily submit award applications electronically. Simply complete the information below and click the SUBMIT button at the bottom of the form. Fields with a red asterisk (*) are required to have data or the form will not be submitted. If you make a mistake, click on the RESET button at the bottom of the form to clear all entries.
If your computer Internet browser has 'cookies' enabled, your last entered data for the Bowling Center, League or Competition Name and Certification#, plus the Secretary or Competition Official Name and Address will be remembered for you the next time you use this form. You can overwrite or change any of the saved data if necessary. Also, the bowler's series total will now be automatically calculated when you enter the three game scores. Note that JavaScripts do have to be enabled for this form to function correctly. Note that if the Bowling Center was saved from last year you may have to re-select the correct center the first time you use this form again this year!
All awards are limited to one award per achievement per season. Additional achievements bowled during the same season will be added to the bowler's record in the National Bowlers' data base (no award issued). For average based achievements use the previous season's average until a current 12-game average is established.
